Required under 49 CFR §391.23
The applicant understands that, in connection with this application for employment, Schnell Transport LLC may conduct investigations and make inquiries regarding information relevant to the applicant’s employment history, driver qualification, safety performance history, background, and other related matters as may be necessary to evaluate the applicant for employment.
The applicant understands that medical-history inquiries will generally be made only after a conditional offer of employment has been extended, when permitted or required by applicable law or regulation.
The applicant understands that information provided regarding current and/or previous employers may be used, and those employers may be contacted, for the purpose of investigating the applicant’s safety performance history as required by 49 CFR §391.23(d) and (e), and driver qualification information as required or permitted under applicable DOT/FMCSA regulations.
The applicant understands that they may have the right to:
- Review certain information provided by current and/or previous employers;
- Request correction of errors in information provided by previous employers, when applicable;
- Request that corrected information be resent to the prospective employer, when applicable; and
- Submit a rebuttal statement if the applicant and previous employer cannot agree on the accuracy of the information.
The applicant understands that false, incomplete, or misleading information provided in the application, during interviews, or in connection with the employment review process may result in disqualification from employment consideration or termination if discovered after employment begins.
The applicant further understands that, if employed, they will be required to comply with all applicable company rules, safety policies, DOT/FMCSA requirements, and other employment-related requirements of Schnell Transport LLC.
This disclosure is provided by Schnell Transport LLC and may be used by the company, its authorized representatives, agents, compliance providers, background screening providers, insurance representatives, or other designated service providers for employment application review, driver qualification, safety compliance, and DOT/FMCSA recordkeeping purposes.
